Programmes et services de soins communautaires à l'intention des collectivités inuites, métisses et des Premières Nations de l'Ontario comprenant 10 centres d'accès à la santé autochtone dirigés par des organismes de soins de santé primaires communautaires autochtones, fournissant une combinaison de modes traditionnels de guérison, de soins primaires, de programmes culturels, de programmes de promotion de la santé, d'initiatives de développement communautaire et de services de soutien social à l'intention des collectivités inuites, métisses et des Premières Nations en régions rurales, nordiques et urbaines.

L'Ontario passe à l'étape suivante de son plan visant à mieux intégrer le système de soins de santé de la province pour mettre fin aux soins de santé de couloir

Vous continuerez d'avoir accès aux soins dont vous avez besoin et aux fournisseurs de soins de santé avec lesquels vous avez établi des relations. Les soins à domicile se poursuivent comme avant et il n'y a aucun changement au placement en foyer de soins de longue durée.
