Description des services: Coordinates the delivery of health care services to the Anishinabe people across the Sioux Lookout region

  • Provides support to youth with developmental disabilities to create a life plan for the transition to life after graduating from high school
  • Assists with various applications such as housing, passport, special services and Ontario Disability Support Pension (ODSP)
  • Advocates on behalf of the family and individual
  • Offers guidance and support for:
    • Community resources
    • Education and learning opportunities
    • Employment and training opportunities
    • Recreation and leisure
    • Volunteer experiences

    Procédé et formulaires: Completed intake assessment required with Transition Facilitator
    Admissibilité / population desservie: First Nation individuals 16-24 years of age, identified with a developmental disability and living in any of the First Nation communities in the Sioux Lookout district

    Individual must:

    • Agree to program by choice

    • Have an identified developmental disability

    • Be leaving school or have been out no more than 2 years

    • Not be receiving accommodation support by the Ministry of Children, Community and Social Services (MCCSS)

    • Have little or no daytime activities appropriate for their strength and need

    • Agree to participate in a planning process, along with family and interested persons, to identify outcomes to be achieved and supported
