Provides support to youth with developmental disabilities to create a life plan for the transition to life after graduating from high schoolAssists with various applications such as housing, passport, special services and Ontario Disability Support Pension (ODSP)Advocates on behalf of the family and individualOffers guidance and support for:- Community resources
- Education and learning opportunities
- Employment and training opportunities
- Recreation and leisure
- Volunteer experiences

Admissibilité / population desservie:
|
First Nation individuals 16-24 years of age, identified with a developmental disability and living in any of the First Nation communities in the Sioux Lookout district
Individual must:
- Agree to program by choice
- Have an identified developmental disability
- Be leaving school or have been out no more than 2 years
- Not be receiving accommodation support by the Ministry of Children, Community and Social Services (MCCSS)
- Have little or no daytime activities appropriate for their strength and need
- Agree to participate in a planning process, along with family and interested persons, to identify outcomes to be achieved and supported
