Description des services: A goal-oriented outpatient neurology rehabilitation service for clients with neurological conditions
  • Helps clients maximize independence and their functional potential
  • Assists clients to independently access community resources and maintain their ability to live at home
  • Provides the option of having individual or group sessions
  • Offers a team of healthcare providers, including: Occupational Therapist, Physiotherapist, Psychological Associate, Rehabilitation Assistant, Social Worker, Speech-Language Pathologist, and Therapeutic Recreation
Services include:
  • Neurology Day Program
  • Neurology Single Discipline Services

Procédé et formulaires: Self-referral or referral by Physician or Nurse Practitioner required
Admissibilité / population desservie:
  • Individuals 15 years of age and older and a resident of NW Ontario

  • Have a Neurological diagnosis (stroke, acquired brain injury, MS, ALS, Parkinson's Disease, or spinal cord injury

  • Require professional intervention

  • Actively participate in therapy for a minimum of one hour per visit

  • Be independent with toileting or have caregiver assistance
