Description des services: Supports the advancement and evolution of Indigenous primary health care service provisions and planning in Ontario

Works with Indigenous primary health care organizations (IPHCOs) across Ontario, including:
  • Aboriginal Health Access Centres (AHACs)
  • Indigenous Interprofessional Primary Care Teams (IPCTs)
  • Indigenous Community Helath Centres (ICHCs), and
  • Indigenous Family Health Teams (IFHTs)
to address the physical, spiritual, emotional and mental wellbeing of First Nations, Inuit and Métis (FNIM) peoples and communities being served
Promotes high-quality care provision through the Model of Wholistic Health and Wellbeing, which is grounded in Anishinaabe teachings such the Seven Grandfathers and the Medicine Wheel, and recognizes wholistic wellbeing values across many Indigenous Nations, including:
  • Haudenosaunee teaching of the Good Mind
  • Mushkegowuk Cree teachings of Wahkohtowin and Miyo-pimâtisiwin
  • Lenape teachings of balance and interdependence
  • Métis understandings of interconnectedness
  • Inuit Qaujimajatuqangit principles of community care and relational responsibility
