Exists to alleviate the personal and social consequences of Alzheimer's disease and other dementias and to promote research
Provides a clinic for early diagnosing of cognitive impairments- Partners with the Sunset Country Family Health Team to offer the memory clinic
- Provides early diagnosis and treatment for problems associated with memory loss
- Team includes Physicians, Nurse Practitioner, Registered Nurse, Registered Practical Nurse, Social Worker, Dietitian, Pharmacist and Client Services Coordinator with Alzheimer Society

- Requires a referral from Physician
- Comprehensive assessment with patient and family members will be booked; appointment lasts approximately two hours
- Package will be provided for patient to complete prior to initial appointment
