The Points North Family Health Team includes: a Family Physician, Nurse Practitioner, Registered Nurse, and a Social Worker- Family Health Team is an approach to primary health care that brings
together different health care providers to coordinate the highest possible quality of care for the patientThe vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues
Assists patients living with Asthma or Chronic Obstructive Pulmonary Disease (COPD) to better manage their disease processConducts spirometry assessments to determine lung functionProvides education and support to help set goals for individual's careOffers smoking cessation counselling if neededProvides referrals to other programs and services
