Description des services: Coordinates the delivery of health care services to the Anishinabe people across the Sioux Lookout region

  • Works in partnership with primary health care providers to offer services for and raise awareness of Hepatitis C virus (HCV) risks, care and treatment
  • Provides a treatment team made up of HCV Treating Physicians, a Treatment Nurse and a Case Coordinator
  • Services and support include:
    • Primarily community based client care
    • Raising awareness of testing options and access to HCV testing
    • Access to care and treatment
    • Access to mental health and addiction counselling
    • Education and awareness of risk factors and prevention

    • Health Care or Social Service providers can call for referral form to refer a client or patient for services; forms can be faxed or sent via electronic medical record

    • Self-referrals also accepted by calling office or toll-free number
