The Points North Family Health Team includes: a Family Physician, Nurse Practitioner, Registered Nurse, and a Social Worker. Family Health Teams allow physicians and other allied health care providers to work as a team to focus on keeping patients healthy.
Provides a monthly diabetes clinic which offers education and services for the prevention, management and treatment of diabetes- Provides case management and screening services
- Offers basic foot care services
- Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
- Advises on developing and implementing an approach to care and management of diabetes
- Communicates patients' concerns to family physician
- Assists individuals in finding appropriate services
- Provides referrals to other health care professionals
