The Points North Family Health Team includes: a Family Physician, Nurse Practitioner, Registered Nurse, and a Social Worker- Family Health Team is an approach to primary health care that brings
together different health care providers to coordinate the highest possible quality of care for the patientThe vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues
Provides a monthly diabetes clinic which offers education and services for the prevention, management and treatment of diabetes- Provides case management and screening services
- Offers basic foot care services
- Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
- Advises on developing and implementing an approach to care and management of diabetes
- Communicates patients' concerns to family physician
- Assists individuals in finding appropriate services
- Provides referrals to other health care professionals
