Helps Ontarians who use insulin or have gestational diabetes pay for the cost of their blood sugar testing supplies The program is funded by the Ministry of Health and Long-Term Care and administered by Diabetes Canada

Admissibilité / population desservie:
|
- Must be an Ontario resident with a valid Ontario Health card
- Must use insulin or have gestational diabetes
- Must not be receiving any other financial assistance for the items being claimed
Note: First claim must be signed by a doctor or nurse practitioner to confirm insulin is being used to manage diabetes or have gestational diabetes
Eligible supplies
- Blood glucose test strips and lancets - 75% reimbursement up to a maximum of $920/year (up to $1227 in receipts can be submitted each year and the program will reimburse 75%, or $920, of the cost)
- Blood glucose meters - 75% reimbursement up to a maximum of $75 once every 5 years
- Talking blood glucose meters for visually-impaired individuals; letter from doctor required to confirm visual impairment - 75% reimbursement up to a maximum of $300 once every 5 years
