Provides support to youth with developmental disabilities to create a transitional plan for life after graduating from high schoolAssists families to identify and access resources through transitional planning is school partnership and into community lifeAssists with applications and interviews such as Developmental Services Ontario (DSO) housing, passport, Ontario Disability Support Pension and access to adult services, Clinical/Surrey PlaceOffers resource planning for accessing further education, employment, training opportunities and community resources
